Abstract (english):
This paper describes the CPR Tutor, a real-time multimodal feedback system for cardiopulmonary resuscitation (CPR) training. The CPR Tutor detects training mistakes using recurrent neural networks. The CPR Tutor automatically recognises and assesses the quality of the chest compressions according to five CPR performance indicators. It detects training mistakes in real-time by analysing a multimodal data stream consisting of kinematic and electromyographic data. Based on this assessment, the CPR Tutor provides audio feedback to correct the most critical mistakes and improve the CPR performance. The mistake detection models of the CPR Tutor were trained using a dataset from 10 experts. Hence, we tested the validity of the CPR Tutor and the impact of its feedback functionality in a user study involving additional 10 participants. The CPR Tutor pushes forward the current state of the art of real-time multimodal tutors by providing: (1) an architecture design, (2) a methodological approach for delivering real-time feedback using multimodal data and (3) a field study on real-time feedback for CPR training. This paper details the results of a field study by quantitatively measuring the impact of the CPR Tutor feedback on the performance indicators and qualitatively analysing the participants' questionnaire answers. Abstract:
Background: With the onset of the COVID-19 pandemic at the beginning of 2020, the crucial role of hygiene in healthcare settings has once again become very clear. For diagnostic and for didactic purposes, standardized and reliable tests suitable to assess the competencies involved in "working hygienically" are required. However, existing tests usually use self-report questionnaires, which are suboptimal for this purpose. In the present study, we introduce the newly developed, competence-oriented HygiKo test instrument focusing health-care professionals' hygiene competence and report empirical evidence regarding its psychometric properties. Methods: HygiKo is a Situational Judgement Test (SJT) to assess hygiene competence. The HygiKo-test consists of twenty pictures (items), each item presents only one unambiguous hygiene lapse. For each item, test respondents are asked (1) whether they recognize a problem in the picture with respect to hygiene guidelines and, (2) if yes, to describe the problem in a short verbal response. Our sample comprised n = 149 health care professionals (79.1 % female; age: M = 26.7 years, SD = 7.3 years) working as clinicians or nurses. The written responses were rated by two independent raters with high agreement (α > 0.80), indicating high reliability of the measurement. We used Item Response Theory (IRT) for further data analysis. Results: We report IRT analyses that show that the HygiKo-test is suitable to assess hygiene competence and that it allows to distinguish between persons demonstrating different levels of ability for seventeen of the twenty items), especially for the range of low to medium person abilities. Hence, the HygiKo-SJT is suitable to get a reliable and competence-oriented measure for hygiene-competence. Conclusions: In its present form, the HygiKo-test can be used to assess the hygiene competence of medical students, medical doctors, nurses and trainee nurses in cross-sectional measurements. In order to broaden the difficulty spectrum of the current test, additional test items with higher difficulty should be developed. The Situational Judgement Test designed to assess hygiene competence can be helpful in testing and teaching the ability of working hygienically. Further research for validity is needed. (DIPF/Orig.)

Abstract (english):
As the field of social and emotional learning has developed and grown over the last decades, so too has the number of different programmes seek-ing to achieve various SEL goals, resulting in a significant body of research on the topic. Meanwhile, there has also been an increase in the number of programmes aimed at developing intercultural competencies. For the pur-pose of developing a new programme for social and emotional learning, we reviewed the key characteristics of existing evidence-based social, emo-tional and intercultural learning programmes, including their theoretical and conceptual backgrounds, content, methodological and organisation-al aspects, and their evaluation. This review has revealed multiple similar-ities between these two approaches (i.e. social and emotional and intercul-tural), presenting a rationale for their integration. (DIPF/Orig.)

Abstract:
This study investigated to what extent multimodal data can be used to detect mistakes during Cardiopulmonary Resuscitation (CPR) training. We complemented the Laerdal QCPR ResusciAnne manikin with the Multimodal Tutor for CPR, a multi-sensor system consisting of a Microsoft Kinect for tracking body position and a Myo armband for collecting electromyogram information. We collected multimodal data from 11 medical students, each of them performing two sessions of two-minute chest compressions (CCs). We gathered in total 5254 CCs that were all labelled according to five performance indicators, corresponding to common CPR training mistakes. Three out of five indicators, CC rate, CC depth and CC release, were assessed automatically by the ReusciAnne manikin. The remaining two, related to arms and body position, were annotated manually by the research team. We trained five neural networks for classifying each of the five indicators. The results of the experiment show that multimodal data can provide accurate mistake detection as compared to the ResusciAnne manikin baseline. We also show that the Multimodal Tutor for CPR can detect additional CPR training mistakes such as the correct use of arms and body weight. Thus far, these mistakes were identified only by human instructors. Finally, to investigate user feedback in the future implementations of the Multimodal Tutor for CPR, we conducted a questionnaire to collect valuable feedback aspects of CPR training. (DIPF/Orig.)

Abstract:
Hintergrund: Neben einem erhöhten Risiko für Komplikationen und Behandlungsfehler führen insuffiziente Übergaben zu Therapieverzögerungen, längeren Krankenhausaufenthalten, erhöhten Kosten und vermehrten Patientenbeschwerden. Die Deutsche Gesellschaft für Anästhesiologie und Intensivmedizin (DGAI) und der Europäische Rat für Wiederbelebung (ERC) empfehlen die Einführung eines Kommunikationsablaufs nach dem SBAR-Konzept. Bisher existieren in Deutschland kaum Anforderungen Patientenübergaben in der Ausbildung zu adressieren. Methoden: Im Rahmen des EU-geförderten Projektes PATIENT wurde eine onlinebasierte Bedarfsanalyse im Querschnittsdesign durchgeführt. In Aachen wurden 237 Medizinstudierende zu 45 Items in drei Abschnitten zum Thema Übergabeprozesse befragt. A: Fertigkeiten (Wichtigkeitsbeurteilung und Selbsteinschätzung); B: Erfahrungen aus der klinischen Praxis (Zustimmung); C: Curriculumsinhalte (Präsenz und Relevanz). Alle Items wurden mittels einer Likert-Skala (0-7) bewertet. Ergebnisse: Die adäquate Durchführung einer Patientenübergabe wurde von Studierenden bei eigener geringer Handlungskompetenz (M = 4,0; SD: ±1,3) durchschnittlich als sehr wichtig beurteilt (M = 6,8; SD: ±0,6). Ein hoher Trainingsbedarf wurde für die Items "Entlassbrief schreiben" sowie "akkurate Übergaben durchführen" erfasst. Den Zusammenhang zwischen adäquaten Patientenübergaben und Patientensicherheit erkennen die Studierenden (M = 6,5; SD: ±0,9). Sie sprechen standardisierten Übergabeprozessen im Curriculum eine hohe Bedeutung zu (M = 6,1; SD: ±1,1). Der Umgang mit medizinischen Fehlern ist aus Sicht der Studierendenden im Curriculum unterrepräsentiert (61,7%). Schlussfolgerung: Es liegt ein Trainingsbedarf für Patientenübergaben vor, insbesondere bei Verlegungen und Entlassungen. Daraus wurden Lernziele definiert, Trainingsmodule entwickelt und am Standort Aachen in die Ausbildung integriert. (DIPF/Orig.)
Abstract (english):
Background: Insufficient handoffs lead to an increase in the risk of complications and malpractice, treatment delays, prolonged hospital stays, costs and patient complaints. The German Society for Anesthesiology and Intensive Care (DGAI) and the European Resuscitation Council (ERC) recommend the implementation of a communication procedure according to the SBAR concept. So far, there have been few curricular requirements in Germany regarding handoffs. Methods: As part of the EU-funded PATIENT project an online-based cross-sectional needs analysis was conducted in three countries. In Aachen, 237 medical students answered 45 items concerning handoffs in three sections: A: skills (relevance and self-assessment), B: clinical experience (agreement), C: curricula content (presence and relevance). Data was recorded using a Likert scale (0-7). Results: The students rated an adequate handoff performance as highly important (M = 6.8; SD: ±0.6) and their own expertise as low (M = 4.0; SD: ±1.3). A high training need was identified for writing discharge letters and performing accurate handoffs. The students were aware of the link between adequate handoffs and patient safety (M = 6.5; SD: ±0.9). They considered standardized handoff procedures as an important curricular component (M = 6.1; SD: ±1.1). From their point of view, the handling of medical errors is underrepresented in the curriculum (61.7 %). Conclusion: A need for handoff training was revealed, especially regarding transfers and discharges. Accordingly, learning objectives were determined and training modules developed and integrated into the curriculum in Aachen. (DIPF/Orig.)

Abstract (english):
Background: Evidence suggests that pooling multiple independent diagnoses can improve diagnostic accuracy in well-defined tasks. We investigated whether this is also the case for diagnostics in emergency medicine, an ill-defined task environment where diagnostic errors are rife. Methods: A computer simulation study was conducted based on empirical data from 2 published experimental studies. In the computer experiments, 285 medical students independently diagnosed 6 simulated patients arriving at the emergency room with dyspnea. Participants' diagnoses (n = 1,710), confidence ratings, and expertise levels were entered into a computer simulation. Virtual groups of different sizes were randomly created, and 3 collective intelligence rules (follow-the-plurality rule, follow-the-most-confident rule, and follow-the-most-senior rule) were applied to combine the independent decisions into a final diagnosis. For different group sizes, the performance levels (i.e., percentage of correct diagnoses) of the 3 collective intelligence rules were compared with each other and against the average individual accuracy. Results: For all collective intelligence rules, combining independent decisions substantially increased performance relative to average individual performance. For groups of 4 or fewer, the follow-the-most-confident rule outperformed the other rules; for larger groups, the follow-the-plurality rule performed best. For example, combining 5 independent decisions using the follow-the-plurality rule increased diagnostic accuracy by 22 percentage points. These results were robust across case difficulty and expertise level. Limitations: Our findings are based on simulated patients diagnosed by medical students. Whether results generalize to clinical practice is currently unknown. Conclusion: Combining independent decisions may substantially improve the quality of diagnoses in emergency medicine and may thus enhance patient safety. (DIPF/Orig.)

Abstract (english):
According to the schema-relatedness hypothesis, new experiences that make contact with existing schematic knowledge are more easily encoded and remembered than new experiences that do not. Here we investigate how real-life gains in schematic knowledge affect the neural correlates of episodic encoding, assessing medical students 3 months before and immediately after their final exams. Human participants were scanned with functional magnetic resonance imaging while encoding associative information that varied in relatedness to medical knowledge (face-diagnosis vs face-name pairs). As predicted, improvements in memory performance over time were greater for face-diagnosis pairs (high knowledge-relevance) than for face-name pairs (low knowledge-relevance). Improved memory for face-diagnosis pairs was associated with smaller subsequent memory effects in the anterior hippocampus, along with increased functional connectivity between the anterior hippocampus and left middle temporal gyrus, a region important for the retrieval of stored conceptual knowledge. The decrease in the anterior hippocampus subsequent memory effect correlated with knowledge accumulation, as independently assessed by a web-based learning platform with which participants studied for their final exam. These findings suggest that knowledge accumulation sculpts the neural networks associated with successful memory formation, and highlight close links between knowledge acquired during studying and basic neurocognitive processes that establish durable memories. SIGNIFICANCE STATEMENT: In a sample of medical students, we tracked knowledge accumulation via a web-based learning platform and investigated its effects on memory formation before and after participants' final medical exam. Knowledge accumulation led to significant gains in memory for knowledge-related events and predicted a selective decrease in hippocampal activation for successful memory formation. Furthermore, enhanced functional connectivity was found between hippocampus and semantic processing regions. These findings (1) demonstrate that knowledge facilitates binding in the hippocampus by enhancing its communication with the association cortices, (2) highlight close links between knowledge induced in the real world and basic neurocognitive processes that establish durable memories, and (3) exemplify the utility of combining laboratory-based cognitive neuroscience research with real-world educational technology for the study of memory. Abstract:
In recent years, we have seen an increasing amount of interest in low-dimensional vector representations of words. Among other things, these facilitate computing word similarity and relatedness scores. The most well-known example of algorithms to produce representations of this sort are the word2vec approaches. In this paper, we investigate a new model to induce such vector spaces for medical concepts, based on a joint objective that exploits not only word co-occurrences but also manually labeled documents, as available from sources such as PubMed. Our extensive experimental analysis shows that our embeddings lead to significantly higher correlations with human similarity and relatedness assessments than previous work. Due to the simplicity and versatility of vector representations, these findings suggest that our resource can easily be used as a drop-in replacement to improve any systems relying on medical concept similarity measures. ULME III gibt Auskunft über den Leistungsstand von Jugendlichen am Ende der beruflichen Ausbildung in 17 ausgewählten Ausbildungsberufen im Bereich übergreifender allgemeiner Grundqualifikationen, metakognitiver Strategien und berufsspezifischer Fachqualifikationen (DIPF/Autor).

Abstract:
Die Hamburger Schulleistungsstudie ULME III umfasste - neben gewerblich-technischen und kaufmännischen Berufen - auch Jugendliche in den Ausbildungsgängen Medizinische/-r und Zahnmedizinische/-r Fachangestellte/-r sowie Friseur/-in. Im Beitrag werden die von Hamburger Berufsschullehrern in Kooperation mit dem Institut für Berufs- und Wirtschaftspädagogik der Universität Hamburg entwickelten berufsspezifischen Fachleistungstests für die drei genannten Ausbildungsberufe vorgestellt und hinsichtlich teststatistischer Gütekriterien ausgewertet. Weiterhin werden die berufsfachlichen Schülerfähigkeiten am Ende der Ausbildung ausführlich berichtet, wobei auf die Lage spezifischer Schülergruppen differenziert eingegangen wird. Die multivariaten Analysen zum Einfluss individueller Schülermerkmale auf die Entwicklung beruflicher Fachkompetenz unterstreichen die substanzielle Bedeutsamkeit allgemeiner kognitiver Grundqualifikationen im Sprach- und Leseverständnis sowie in grundlegenden Rechenfertigkeiten. Abstract:
Das besondere Auswahlverfahren für medizinische Studiengänge, das zum Wintersemester 1986/87 eingeführt wurde und die Teilnahme am "Test für medizinische Studiengänge" zur Pflicht macht, versucht durch Kombination verschiedener Zulassungskriterien einen Kompromiss zwischen hoher Validität einerseits, Eröffnung reeller Chancen für alle Bewerber andererseits zu erzielen. Seine spezifischen Entscheidungsregeln, die zum Beispiel einem Abgelehnten beliebig häufige Wiederbewerbungen ermöglichen und dadurch ein Test-Equating sinnvoll erscheinen lassen, können mit traditionellen Modellen nicht zureichend erfasst werden. Ein spezielles Simulationsmodell, das Entwicklung und Auswirkungen von Selektionsraten, Leistungsanforderungen und anderen Parametern beschreibt, wird vorgestellt und auf mehrere Szenarien angewandt. Seine Prognosen stimmen mit vorliegenden Realdaten überein. Sie lassen ein Absinken der Leistungsanforderungen erwarten und bestätigen die chancenausgleichende Wirkung des Verfahrens. (DIPF/Orig.)
