Abstract: Older patients, suffering from numerous diseases and taking multiple medications are the rule rather than the exception in primary care. A manifold of medical conditions are often associated with poor outcomes, and their multiple medications raise additional risks of polypharmacy. Such patients account for most healthcare expenditures. Effective approaches are needed to manage such complex patients in primary care. This paper describes the results of a scoping exercise, including a two-day workshop with 17 professionals from six countries, experienced in general practice and primary care research as well as epidemiology, clinical pharmacology, gerontology and methodology. This was followed by a consensus process investigating the challenges and core questions for multimorbidity research in primary care from a clinical perspective and presents examples of the best research practice. Current approaches in measuring and clustering multimorbidity inform policy-makers and researchers, but research is needed to provide support in clinical decision making. Multimorbidity presents a complexity of conditions leading to individual patient's needs and demanding complex processes in clinical decision making. The identification of patterns presupposes the development of strategies on how to manage multimorbidity and polypharmacy. Interventions have to be complex and multifaceted, and their evaluation poses numerous methodological challenges in study design, outcome measurement and analysis. Overall, it can be seen that complexity is a main underlying theme. Moreover, flexible study designs, outcome parameters and evaluation strategies are needed to account for this complexity.

Abstract: The brain shrinks with age, but the timing of this process and the extent of its malleability are unclear.
We measured changes in regional brain volumes in younger (age 20-31) and older (age 65-80) adults
twice over a 6 months period, and examined the association between changes in volume, history of
hypertension, and cognitive training. Between two MRI scans, 49 participants underwent intensive practice
in three cognitive domains for 100 consecutive days, whereas 23 control group members performed
no laboratory cognitive tasks. Regional volumes of seven brain structures were measured manually and
adjusted for intracranial volume. We observed significant mean shrinkage in the lateral prefrontal cortex,
the hippocampus, the caudate nucleus, and the cerebellum, but no reliable mean change of the prefrontal
white matter, orbital-frontal cortex, and the primary visual cortex. Individual differences in change were
reliable in all regions. History of hypertension was associated with greater cerebellar shrinkage. The cerebellum
was the only region in which significantly reduced shrinkage was apparent in the experimental
group after completion of cognitive training. Thus, in healthy adults, differential brain shrinkage can be
observed in a narrow time window, vascular risk may aggravate it, and intensive cognitive activity may
have a limited effect on it.

Abstract (english): We examined in this study the hypothesis that cognitive resources are more closely linked with out-of-home behavior than motivational resources. A cognitively heterogeneous sample of 222 older adults aged 59-91 years (M = 72.7; SD = 6.2), including 146 cognitively healthy persons and 76 persons with mild cognitive impairment - recruited in the German and Israeli arm of the SenTra project - was used for the analysis. Out-of-home behavior was assessed by means of global positioning system technology (time out of home; number of nodes visited) as well as by questionnaire (out-of-home activities). Mini-Mental State Examination and trail-making tests A and B were used to assess cognitive resources. Well-being, depression, and environmental mastery were assessed as motivational resources. Findings at the zero-order and latent variable levels confirmed that cognitive resources were more closely linked with out-of-home behavior than motivational resources. Findings support the view that well-being-related motivations to exert out-of-home behavior may become less important in old age because of the increasing cognitive resources required by such behavior.

Abstract: Whether received social support matches the actual needs of the recipient is a largely overlooked aspect
in research on associations of support and well-being. In particular, studies that investigate the match of
needed and received support from a within-person perspective are rare. Therefore, we investigated the
daily within-person relationship of well-being and the balance of needed and received social support in a
German Sample of 79 younger (23e34 years) and 88 older adults (68e83 years). Health complaints and
negative affect were predicted with linear effects of received and needed emotional support and
quadratic effects of the balance (i.e., difference) of these two aspects using multilevel modeling of selfreports
over 20 days. The predicted beneficial association between a match of needed and received
support (i.e., a support balance) and well-being was observed among younger adults. Needed support
was associated with more health complaints and negative affect on the same day. The match of needed
and received support is important for well-being, particularly in younger adults. Future research should
account for support needed in research on received support and shed more light into the processes
underlying these short-term within-person relationships of social support and well-being.

Title: Le modèle hiérarchique de diffusion.

Abstract (english): The importance of socioeconomic status (SES) for psychological functioning over the life span is increasingly acknowledged in psychological research. The Reserve Capacity Model by Gallo and Matthews (2003) suggests that SES is not only linked to physical health but also to the experience of positive and negative emotions. Moreover, due to differential amounts of psychosocial resources, cross-domain associations between emotions and health might differ according to SES. The present study examined age-related developments in positive affect (PA), negative affect (NA), and physical health, as well as dynamic associations between health and emotions in the second half of life. We looked at differences in these trajectories and their interrelationships according to education as one aspect of SES. We used data of up to three waves spanning 12 years from the nationally representative German Ageing Survey (N 3,847, AgeT1 40-85 years). Applying multiple-group dual change score models, we found differential age-related change in PA and physical health, but not in NA, in two groups differing in level of education. NA did only predict change in physical health in low-educated individuals, whereas physical health was equally strongly related to change in PA in both education groups. These results indicate that SES not only affects changes in physical health and emotional functioning but also their interrelationships.

Abstract (english): We investigated working memory updating performance in younger and older adults before, during, and after 100-day practice. Performance to presentation time (PT) relation was fitted to a negatively accelerated logistic function. Relative to younger adults, older adults showed lower asymptotes at pretest and posttest, and shallower slopes at pretest. Older adults practicing the task with fast PT gained less than older adults practicing the task with slow PT, probably reflecting the persistent use of a selective strategy throughout the 100-day practice period in the fast PT group. These results have implications for designing and evaluating age-comparative working memory training programs.
