Abstract (english):
Valid and reliable standardized assessment of nursing competencies is needed to monitor the quality of vocational education and training (VET) in nursing and evaluate learning outcomes for care work trainees with increasingly heterogeneous learning backgrounds. To date, however, the modeling of professional competencies has not yet evolved into procedures that would meet large-scale assessment (LSA) standards in VET. To empirically test a proposed structural model for client-directed nursing competence and to estimate psychometric properties of a newly developed video- and computer-based test (CBT) to inform subsequent LSA in nursing VET, 402 final-year nursing students from 24 German schools responded to a 77 item CBT. Multi-dimensional IRT modeling was employed to test the subdomain structure and estimate students' competencies in geriatric nursing. The standardized CBT measures nursing students' client-directed care competence with acceptable precision (WLE= 0.76) and does so across the whole range of observed proficiency levels. Structural validity was supported by substantive contributions of test items from all proposed process-oriented subdomains, practice field scenarios, as well as items with and without reference to emotional demands. However, it was not possible to empirically separate the diagnostic, practical or communicative subdomains, probably reflecting parallel, recursive and hierarchical care processes in complex care situations. On average, students in our sample attained 45 % of the maximum test score so it is a demanding assessment of nursing competence. An extensively piloted, valid and reliable CBT is suggested to assess nursing students' client-directed care competencies at the end of the third year of the VET program. (DIPF/Orig.)

Abstract:
Unter Verwendung von Mehrebenen-Regressionsmodellen werden in diesem Beitrag Zusammenhänge zwischen von Altenpflegeschülerinnen und -schülern wahrgenommenen betrieblichen Ausbildungsbedingungen und der Testleistung in einem psychometrischen Test berufsfachlicher Handlungskompetenz im unmittelbar bewohnerbezogenen Bereich der Pflege älterer Menschen untersucht. Neben in der Literatur aufzufindenden Qualitätsmerkmalen betrieblicher Ausbildung wurden inhaltliche Aufgabenbereiche der betrieblichen Praxiseinrichtungen sowie - vor dem Hintergrund theoretischer Annahmen zur Lernwirksamkeit von zwischen Betrieb und Schule abgestimmten Ausbildungsinhalten - deren Kongruenz zu Inhaltsbereichen der Altenpflegeschulen im Zusammenhang mit der Testleistung analysiert. In einem querschnittlichen Design wurden hierzu 402 Absolventinnen und -absolventen der Pflegeausbildung aus 24 Schulklassen am Ende der Ausbildung befragt und getestet. Die Ergebnisse erweisen sich nur in Teilen als erwartungskonform. Die Befunde legen nahe, gegenüber globalen Merkmalen betrieblicher Ausbildungsqualität eher proximale Merkmale der Ausbildung in den betrieblichen Praxiseinrichtungen, wie den Umfang erfahrener inhaltlicher Aufgabenbereiche und die qualitative Befassung mit diesen spezifischen Aufgabenbereichen, verstärkt zu beleuchten. Abstract (english):
Using multi-level regression models, in this contribution we study the relationship between practical training conditions and the test score of geriatric nursing students in a psychometric competence test on immediately client-related geriatric nursing. Apart from features of high quality practical training found in existing literature, task content in the practical training sites as well as - on the background of theoretical assumptions on the effects of the coordination of content between practical training sites and the schools on learning - congruency of training site tasks with content taught at schools are being analyzed. In a cross-sectional design, 402 geriatric nursing students from 24 classes were questioned and tested at the end of VET. Test results are only partially congruent with expectations. The findings suggest to pay more attention to the study of proximal aspects of training at the practical training sites rather than global estimates of training quality, like task content and the qualitative aspects of confrontations with specific task content. Abstract:
Global aging, particularly in Japan and Europe, an associated shortage of qualified healthcare personnel, and a general preference for living at home, have all sparked an interest in the possibilities offered by robot therapy in improving quality of life. To date, however, both evidence-based research and psychological theory building on whether, and if so how and why, robots can influence the life of the elderly are rather sparse and scattered across many scientific disciplines and countries. The present paper argues in favor of a psychological, cross-cultural framework for evidence-based analyses of emotional robots. Against the background of this framework, we present building blocks for an evidence-based approach to the assessment of emotional robots and discuss interactions between evidence- and deployment-based research.

Abstract:
Older patients, suffering from numerous diseases and taking multiple medications are the rule rather than the exception in primary care. A manifold of medical conditions are often associated with poor outcomes, and their multiple medications raise additional risks of polypharmacy. Such patients account for most healthcare expenditures. Effective approaches are needed to manage such complex patients in primary care. This paper describes the results of a scoping exercise, including a two-day workshop with 17 professionals from six countries, experienced in general practice and primary care research as well as epidemiology, clinical pharmacology, gerontology and methodology. This was followed by a consensus process investigating the challenges and core questions for multimorbidity research in primary care from a clinical perspective and presents examples of the best research practice. Current approaches in measuring and clustering multimorbidity inform policy-makers and researchers, but research is needed to provide support in clinical decision making. Multimorbidity presents a complexity of conditions leading to individual patient's needs and demanding complex processes in clinical decision making. The identification of patterns presupposes the development of strategies on how to manage multimorbidity and polypharmacy. Interventions have to be complex and multifaceted, and their evaluation poses numerous methodological challenges in study design, outcome measurement and analysis. Overall, it can be seen that complexity is a main underlying theme. Moreover, flexible study designs, outcome parameters and evaluation strategies are needed to account for this complexity.

[Der Beitrag berichtet über eine] Machbarkeitsstudie, mit der in acht europäischen Ländern geprüft wurde, ob ausgewählte Ausbildungsberufe in den Bereichen Technik, Wirtschaft und Verwaltung sowie Gesundheit und Soziales jeweils hinreichende inhaltliche Überschneidungen zwischen den Ländern aufweisen, so dass ein "Berufsbildungs-PISA" durchgeführt werden könnte. ... Zumindest zwei Merkmale kennzeichnen jedoch berufliche Ausbildungssysteme, die die Durchführung eines Berufsbildungs-PISAs erschweren: Zum einen die unterschiedlichen Zuschnitte und Denominationen von Berufen, zum anderen die in den Ländern jeweils unterschiedliche Zuordnung der Ausbildung für diese Berufe zwischen nichtakademischer und akademischer Ausbildung. Wie nachfolgend gezeigt wird, gilt dies insbesondere für den Bereich Gesundheit und Soziales, wo die Auswahl der Ausbildungsberufe sich in den Ländern als deutlich schwieriger erwies als etwa im Ausbildungsbereich Technik. Hier wurde für Deutschland nach der systematischen Prüfung verschiedener Optionen der Beruf Altenpfleger/in für die Machbarkeitsstudie ausgewählt. (DIPF/Orig.)

Abstract:
Einigkeit herrscht bei allen nationalen sozial- und bildungspoltischen Akteuren, dass mit den Sozial- und Gesundheitsberufen ein expandierendes Beschäftigungsfeld vorliegt, für das Regelungsbedarf besteht. Die gesetzlichen Möglichkeiten und Folgen waren das Thema einer diesbezüglichen Konferenz, in die sich der o.g. Beitrag einfügt. In ihm wird im Vergleich zu den Niederlanden, Grossbritannien und Frankreich die Pflegeausbildung dargestellt, die sich auch in Deutschland nicht nur der Vermittlung von fachlichen, sondern auch Europakompetenzen zuwenden muss. Eine wichtige Ergänzung für die an qualifizierter Pflegeausbildung Interessierten ist die beigefügte Bibliografie zu "Entscheidungen der EU zur Anerkennung von Abschlüssen in den Pflegeberufen".
Abstract (english):
All sociopolitical and politico-educational protagonists agree on the fact that the occupations within the social and health services offer an expanding occupational field that needs to be regulated. The legal possibilities and consequences were the topic of a corresponding conference in which the above-mentioned contribution fits. It portrays the training of caring and nursing personnel by way of comparison with the Netherlands, Great Britain and France, and shows that this training must turn towards imparting Europe-related competences instead of only imparting specialized job-related competences in Germany as well. An important supplement for those interested in a qualified training of caring and nursing personnel is given by the attached bibliography on "decisions of the EU on the recognition of training certificates for caring and nursing personnel [Entscheidungen der EU zur Anerkennung von Abschlüssen in den Pflegeberufen]".
