Abstract:
Background: Digital phenotyping promises to unobtrusively obtaining a continuous and objective input of symptomatology from patients' daily lives. The prime example are bipolar disorders, as smartphone parameters directly reflect bipolar symptomatology. Empirical studies, however, have yielded inconsistent findings. We believe that three main shortcomings have to be addressed to fully leverage the potential of digital phenotyping: short assessment periods, rare outcome assessments, and an extreme fragmentation of parameters without an integrative analytical strategy. Methods: To demonstrate how to overcome these shortcomings, we conducted frequent (biweekly) dimensional and categorical expert ratings and daily self-ratings over an extensive assessment period (12 months) in 29 patients with bipolar disorder. Digital phenotypes were monitored continuously. As an integrative analytical strategy, we used structural equation modelling to build latent psychopathological outcomes (mania, depression) and latent digital phenotype predictors (sleep, activity, communicativeness). Outcomes: Combining gold-standard categorical expert ratings with dimensional self and expert ratings resulted in two latent outcomes (mania and depression) with statistically meaningful factor loadings that dynamically varied over 299 days. Latent digital phenotypes of sleep and activity were associated with same-day latent manic psychopathology, suggesting that psychopathological alterations in bipolar disorders relate to domains (latent variables of sleep and activity) and not only to specific behaviors (such as the number of declined incoming calls). The identification of latent psychopathological outcomes that dimensionally vary on a daily basis will enable to empirically determine which combination of digital phenotypes at which days prior to an upcoming episode are viable as digital prodromal predictors. Abstract (english):
Children with reading and/or spelling disorders have increased rates of behavioral and emotional problems and combinations of these. Some studies also find increased rates of attention-deficit-/hyperactivity disorder (ADHD), conduct disorder, anxiety disorder, and depression. However, the comorbidities of e.g. arithmetic disorders with ADHD, anxiety, and depression have been addressed only rarely. The current study explored the probability of children with specific learning disorders (SLD) in reading, spelling, and/or arithmetic to also have anxiety, depression, ADHD, and/or conduct disorder. The sample consisted of 3014 German children from grades 3 and 4 (mean age 9;9 years) who completed tests assessing reading, spelling as well as arithmetic achievement and intelligence via a web-based application. Psychopathology was assessed using questionnaires filled in by the parents. In children with a SLD we found high rates of anxiety (21%), depression (28%), ADHD (28%), and conduct disorder (22%). Children with SLD in multiple learning domains had a higher risk for psychopathology and had a broader spectrum of psychopathology than children with an isolated SLD. The results highlight the importance of screening for and diagnosing psychiatric comorbidities in children with SLD.

Abstract (english):
Objective: To study normative data and psychometric properties of the Thai Strengths and Difficulties Questionnaire (SDQ; parent-, teacher-, and self-rated forms), which addresses positive and negative aspects of children's and adolescents' behaviour and generates clinically relevant scale scores. Materials and Methods: Using multistage random cluster sampling method, data were collected in 13 provinces from parents, teachers, and as self-reports of 9,491 children aged between 5 and 16 years. Evaluation methods included scale reliability analyses (Cronbach s alpha), correlations with age and among scales, testing for gender effects, and comparing urban and rural regions. A factor analysis examines the specific scale structure of the Thai parent-rated SDQ. Bandings are recommended to identify normal, borderline, and abnormal score ranges. Results: Problem scores were higher than those observed in Western countries, stressing the necessity to establish national norms. Thai SDQ norms identify probable behaviour problems if the total difficulties score is 19-40 in the parent-rated form,17-40 in the teacher form, and/or 19-40 in the self-report. Internal reliabilities were satisfactory for all but one subscale. Age and gender effects on SDQ scores as well as correlations between subscales were well in line with the English original and its many other translated versions. Conclusion: The Thai SDQ was shown to possess sufficiently favourable psychometric properties. Thus, this instrument promises to be a useful assessment and screening tool, as in other parts of the world. Ongoing validation studies and cross-cultural comparisons will provide further culture-specific findings.
